To register as a new Patient please print off, complete and sign a Form GMS1 (pdf) (Family doctor services registration) and a New Patient Questionnaire for adults and for children. Please bring them in to reception with a form of photo ID and a separate proof of address (ie utility bill/passport/photo driving licence).

All residents of Burley-in-Wharfedale are welcome to join our lists, as are those who live before the traffic lights in Ilkley by the Crescent Hotel and before the Fox restaurant in Menston.

Before being accepted on to the practice list it is necessary to complete a short questionnaire requesting details of any previous medical history.
