There are two ways to register as a new Patient:

1. please print off, complete and sign a Form PRF1 (pdf) (Family doctor services registration). Please bring it in to reception with a form of photo ID and a separate proof of address (ie utility bill/passport/photo driving licence) or
2. use this link to register online - https://gp-registration.nhs.uk/B83019

If you're registering for a child please also complete our new child registration questionnaire.

All residents of Burley-in-Wharfedale are welcome to join our lists, as are those who live before the traffic lights in Ilkley by the Crescent Hotel and before the Fox restaurant in Menston. The map below highlights our catchment area.

If you are visiting the area and need to see a doctor in the practice please complete and sign a Form GMS3/99 (pdf) (Temporary services) and bring it in to reception.
